Medical
You may choose from two different medical plans: a comprehensive major medical plan or a consumer-directed health plan that allows you to manage your medical costs. The company pays a portion of the premium for full-time employees and their families.
Dental
DeCare provides a dental plan that covers preventative and basic services. The plan offers benefits whether you go to a provider in or out of the network. To maximize your savings and reduce your out-of-pocket costs, we encourage you to utilize an in-network provider. The Company pays the full cost of the plan for full-time employees and their families.
SelectAccount
The plan allows you to receive a portion of your paycheck as a tax-free fund to cover eligible health and dependent daycare expenses. The maximum annual contribution for a Health Care Expense Account is $3,000 and the maximum annual contribution for a Dependent Day Care Account is $5,000.
DeCare Dental provides you with life insurance if you are a full-time employee scheduled to work at least 32 hours per week.
Basic Term Life Insurance
The Company provides group life insurance coverage equal to one time your annual base salary or $50,000, whichever is greater. This coverage also includes an equal amount of accidental death and dismemberment (AD&D) benefits.
Supplemental & Dependent Life Insurance Coverage
Voluntary life insurance coverage is also available for yourself and eligible dependents.
401(k)
The Company offers a Safe Harbor 401(k) Plan that allows employees to make contributions up to the annual maximum amount per year to their retirement plan. The Company will match 100% on the first 3% of the employee’s deferral and 50% on the next 2% of the employee’s deferral.
Employees are eligible to enroll on their hire date. Newly hired employees are automatically enrolled in the plan at a contribution rate of 3%, unless the employee elects to opt out of the plan.
There are rollover provisions, as well as a loan program available. Vesting is full and immediate on the matching contribution of the first 5% of salary.
In addition, the Company offers a discretionary 401(k) match, which is based on attainment of company goals. This match will be determined at the end of the plan year. The vesting period for the discretionary match is three years.
Short-term Disability (STD)
STD, available at no cost to eligible employees, provides 80% of normal base pay (up to a maximum of $1,000 a week). Employees are covered on the first day of the month following 30 calendar days of continuous service. If you become disabled, you use PTO for the first five days (40 hours) before eligible disability benefits begin.
Long-term Disability (LTD)
LTD, available at no cost to employees working 32 hours per week or more, provides 60% of normal base salary (up to a maximum of $15,000 a month), beginning 180 calendar days after the onset of the disability.
The paid time off program allows employees to manage their own time off for vacation and occasional absences from the scheduled workday while protecting the employee’s income.
PTO may be used for any of the following reasons: vacation, personal or family illness, medical or dental appointments, severe weather, child care/elder care, personal business or observance of religious holidays.
PTO is accrued for all employees on a biweekly basis starting at 5.4 hours for non-exempt (hourly) employees and 6.93 hours for exempt (salaried) employees. It is prorated for employees who work 20 to 32 hours per week.

Holidays
There are nine paid holidays each year, which may include floating holidays (employee choice). Holidays are prorated for employees who work 20 to 32 hours per week.
Employees are paid every other week (26 pay periods). The company offers a direct-deposit payroll option.
All employees are eligible for financial assistance for post-secondary education, licensing, technical training, seminars and workshops.
After six months of employment, financial assistance is provided to employees who voluntarily undertake and successfully complete training/education programs that are relevant to their work or the general business activity of the company. For post-secondary education, employees are reimbursed up to $2,500 a calendar year for under-graduate studies and $4,000 a calendar year for graduate studies. Employees must be working towards a job/business related degree.
Wellness Program
The DeCare Wellness Program offers activities, events and education and training programs designed to encourage employees to assume greater responsibility for their health choices and to promote a healthy lifestyle. It also aims to heighten employee awareness of healthy behaviors and to provide employees an opportunity to become involved in healthy activities.
